System Navigation

A community needs assessment carried out in 2020 identified a key gap in the services and supports for the rare, genetic and undiagnosed community in WA, that being a lack of coordination of information and care in the service system. On the back of this finding, we are trialling a system navigation pilot program for people living with neuromuscular conditions and their families.

After 12 months of extensive research and resource development (with the help of our community consultation committee), our pilot system navigation is now available to people living in Western Australia who:

  • have a neuromuscular condition (NMC)

  • have symptoms of an NMC

  • are awaiting a diagnosis

  • are newly diagnosed

  • who have been previously diagnosed but have never sought system navigation and/or services support before.

System navigation includes many systems, not just health and disability, such as NDIS, Centrelink, advocacy, equipment, transport, education, housing and mental health.

This service is free of charge, as we believe in providing Support for Life.
